Welcome to the Terms and Conditions page for the CARE Sangam Card. This page outlines the rights,
responsibilities, and guidelines associated with the use of the Sangam Card. By using this health
card,
you agree to be bound by these terms. Please read them carefully to understand the scope of
benefits,
eligibility criteria, usage policies, and any limitations that may apply.
User Eligibility Criteria
- The primary cardholder must be 18 years of age or older; minors can join as beneficiaries with
their guardian's consent.
- The primary cardholder must present a Government-issed Valid ID proof at registration to avail
any of the services
mentioned under Sangham Membership.
- The address on the submitted proof must match the address provided during enrolment. Expired,
illegible, or mismatched documents will result in cancellation of membership and/or a request to
resubmit.
- If, at any point after activation, it is found that the registered documents are fraudulent or
do not match the user, CARE Hospitals reserves the right to suspend or terminate the membership
immediately.
Usage Policies and Scope of Benefits
- The card is non-transferable and issued in the name of the Primary user.
- Discounts vary by service, hospital, and treatment type; exclusions may apply.
- Only one free consultation with a general physician per card is allowed until the card's
validity expires.
- No two offers can be combined under any circumstances.
- The card is valid until 31st December 2026 and requires renewal for continued benefits.
