Sleep Apnea

Sleep apnea is a serious sleep disorder that occurs when a person’s breathing is interrupted during sleep. People with untreated sleep apnea stop breathing repeatedly during their sleep, sometimes hundreds of times during the night.

There are two types of sleep apnea: obstructive and central. Obstructive sleep apnea is the more common of the two. Obstructive sleep apnea occurs as repetitive episodes of complete or partial upper airway blockage during sleep. During an apneic episode, the diaphragm and chest muscles work harder as the pressure increases to open the airway. Breathing usually resumes with a loud gasp or body jerk. These episodes can interfere with sound sleep, reduce the flow of oxygen to vital organs, and cause heart rhythm irregularities.

Causes and Symptoms

While just about anybody can become afflicted with obstructive sleep apnea, there are factors that make getting obstructive sleep apnea more likely:

  1. Weight- In many cases a person’s body weight is directly linked to having obstructive sleep apnea. People who are overweight or obese are more likely to have sleep apnea than those that maintain a healthy weight. Sleep apnea can often be caused by excess fatty tissues that become built up in the neck and throat. This can lead to restrictions in airflow as the upper respiratory system’s pathway is narrowed or pinched off during sleep.
  2. Age- As people age their muscles begin to lose muscle tone. This is also true of the muscles in the throat. As throat muscles lose definition, they become weaker and more likely to collapse into the airways during sleep.
  3. Enlarged tonsils or adenoids are the leading cause of obstructive sleep apnea in children but can also affect adults who never had a tonsillectomy when they were younger.
  4. Natural causes- Some people can be genetically predisposed to having a narrower throat or may have an enlarged tongue that falls back into their airway. If your family has a history of OSA you are more likely to have it yourself.
  5. Frequent alcohol use- Alcohol relaxes the muscles in the body, and this includes the throat muscles as well which may relax to the point of blocking the airway during sleep.
  6. Smoking- Smoke is an irritant to the lungs, throat, and esophagus. It can cause inflammation and fluid retention in the upper airways that can impede airflow.
  7. Often the first signs of OSA are recognized not by the patient, but by the bed partner. Many of those affected have no sleep complaints. The most obvious sign of sleep apnea is chronic, loud, and ongoing snoring. Pauses will occur during the snoring, and gasping or choking noises may follow pauses.

Other symptoms of sleep apnea may include –
Daytime sleepiness or fatigue

  1. Restlessness during sleep, frequent nighttime awakenings
  2. Sudden awakenings with a sensation of gasping or choking
  3. Dry mouth or sore throat upon awakening
  4. Cognitive impairment, such as trouble concentrating, forgetfulness, or irritability
  5. Mood disturbances (depression or anxiety)
  6. Night sweats
  7. Sexual dysfunction
  8. Headaches
Diagnosis and Treatment

Your doctor will evaluate your symptoms and then decide if you should be referred to a sleep specialist. Part of your examination will be the checking of your nose, mouth, and throat for large or additional tissues. In children, the problem could well be enlarged tonsils. A sleep study is a series of tests to measure the quality of your sleep and how your body is reacting to sleep problems. The results of these tests will help your doctor determine if you do in fact have a sleep disorder, and its severity. The tests conducted in sleep studies are the most accurate in diagnosing sleep apnea.

Sleep apnea is treated with lifestyle changes, mouthpieces, breathing devices, and surgery. Medicines typically aren’t used to treat the condition.

The goals of treating sleep apnea are to:

  1. Restore regular breathing during sleep
  2. Relieve symptoms such as loud snoring and daytime sleepiness

Treatment may improve other medical problems linked to sleep apnea, such as high blood pressure. Treatment also can reduce your risk for heart disease, stroke, and diabetes.
