Lumbar spondylosis, also known as spinal osteoarthritis, is a degenerative spine condition that develops gradually, being more common in older persons. It occurs due to the wear-and-tear of the bones that happens from normal everyday actions. The lower spine is composed of disc-like structures that are cushioned by soft gel-like sections in between them. The purpose of these sections is to promote flexibility and absorb the load of stress applied to the vertebra. When a crack or stress fracture appears in this part of the body, it is known as lumbar spondylosis.

Causes and Symptoms

Causes for lumbar spondylosis include hereditary back problems. People who are born with a defect in the pars interarticularis bone in the spine (a condition called spondylolysis) are at increased risk of lumbar spondylosis. Other risk factors include a history of repetitive trauma or hyperextension of the lower back or lumbar spine. Athletes such as gymnasts, weight lifters, and football linemen who have large forces applied to the spine during extension are at greater risk for developing lumbar spondylosis.

In many cases, patients with lumbar spondylosis do not have any obvious symptoms. The conditions may not even be discovered until an x-ray is taken for an unrelated injury or condition.

When symptoms do occur, the most common symptom is lower back pain. This pain may:

  1. Feel like a muscle strain
  2. Radiate to the buttocks and back of the thighs
  3. Worsen with activity and improve with rest

Muscle spasms may lead to additional signs and symptoms, including:

  1. Back stiffness
  2. Tight hamstrings (the muscles in the back of the thigh)
  3. Difficulty standing and walking
Diagnosis and Treatment

Your doctor will first go over your medical history as well as your family’s. They will then continue to perform a physical examination, testing for a range of motion and checking to see if there is any pain when pressure is applied to the affected area. A neurological exam will also allow the doctor to assess nerves, muscle strength, and reflexes. This initial assessment will need to be complemented by more definitive diagnostic testing, which in the case of lumbar spondylosis, comes in the form of image testing.

Initial treatment is almost always nonsurgical in nature. Most patients with mild spondylolysis will improve with nonsurgical or conservative treatment. The most common forms of conservative treatment for mild cases of spondylosis include:

  1. Chiropractic care
  2. Physical therapy
  3. Exercises and stretches
  4. Yoga
  5. Pain medication
  6. Steroid epidural

The person should take a break from sports and other activities until the pain subsides. An over-the-counter non-steroidal anti-inflammatory drug (NSAID), such as ibuprofen or naproxen might be recommended to help reduce pain and inflammation. Stronger medications might be prescribed if the NSAIDs do not provide relief. Epidural steroid injections — in which medication is placed directly in the space surrounding the spine — might also help reduce inflammation and ease pain.
Stabilization exercises are the mainstay of treatment. These exercises strengthen the abdominal and/or back muscles, minimizing bony movement of the spine. Generally, eight to 12 weeks of aggressive daily treatment with stabilization exercises are needed to achieve clinical improvement.
Surgery might be only necessary if the vertebra continues to slip or if the pain is not relieved by conservative treatment and begins to interfere with daily activities. The main goals of surgery for lumbar spondylosis are to relieve the pain associated with an irritated nerve, to stabilize the spine where the vertebra has slipped out of place, and to increase the person’s ability to function normally.
