Metastatic Brain Cancer

Metastatic brain cancer, also called secondary brain cancer, occurs when cancer spreads to the brain from another part of the body. It’s about ten times more common than primary brain cancer, which starts in the brain.
Brain tumours that spread are classified based on the location of the tumour in the brain, the type of tissue involved, the original location of the tumour, and other factors. Rarely, a tumour can spread to the brain from an unknown location. This is called cancer of unknown primary (CUP) origin.

Metastatic brain tumours occur in about one-fourth of all cancers that spread through the body. They are much more common than primary brain tumours (tumours that start in the brain) and occur in 10 – 30% of adult cancers.

Causes and Symptoms

Many tumour or cancer types can spread to the brain. The most common are:

  1. Bladder cancer
  2. Breast cancer
  3. Certain sarcomas
  4. Germ cell tumours
  5. Kidney cancer
  6. Lung cancer
  7. Melanoma

Some types of cancers rarely spread to the brain, such as colon cancer or prostate cancer.

Common signs and symptoms

  1. Headaches
  2. Seizures
  3. Weakness in the arms or legs
  4. Loss of balance
  5. Memory loss
  6. Speech disturbance

  1. Behavior and personality changes
  2. Blurred vision/vision disturbance
  3. Numbness
  4. Hearing loss
Diagnosis and Treatment

An examination shows brain and nervous system (neurologic) changes based on where the tumour is in the brain. Signs of increased pressure in the skull are also common. Some tumours may not show signs until they are very large. Then, they cause a very quick decline in nervous system function.

The original (primary) tumour may be found by examining tumour tissues from the brain.

Tests may include:

  1. Cerebral angiography
  2. Chest x-ray; mammogram; CT scans of the chest, abdomen, and pelvis to find the original tumour site
  3. CT scan or MRI of the brain to confirm the diagnosis and identify the tumour location (MRI is usually better for finding tumours in the brain)
  4. EEG
  5. Examination of tissue removed from the tumour during surgery or CT scan-guided biopsy to confirm the type of tumour
  6. Lumbar puncture (spinal tap)

The danger of brain metastases is the space they take up in the brain and the pressure they put on surrounding tissue. This pressure can cause the symptoms associated with brain lesions, such as headaches, speech difficulties, seizures, nausea/vomiting, weakness of a limb, or visual disturbances. The goal of initial therapy is to relieve some of this pressure on the brain tissue by decreasing swelling using medications called corticosteroids (dexamethasone, prednisone), either orally or through an intravenous (IV) line. Some patients may see relief of symptoms quickly after starting steroids, however this does not mean the tumour is gone. If patients experience seizures because of their brain metastases, they may also receive anti-seizure medications to prevent further seizures.
For patients with a single brain lesion, surgery may be a good option, especially if the tumour is under control in the rest of the body. However, the lesion must be in an area of the brain where it is safe to operate. Radiation therapy may be used after surgery to prevent tumours from spreading to other areas of the brain or it may be used instead of surgery.
