Liver transplantation is indicated for patients with end-stage liver disease from any cause including alcohol-related cirrhosis, chronic hepatitis B or C, NASH cirrhosis, autoimmune liver disease, Wilson's disease, or biliary conditions and for hepatocellular carcinoma within Milan criteria. Cadaveric transplants are by nature time-constrained. When a compatible organ becomes available, the recipient must be assessed and prepared for surgery within hours. Liver transplantation typically takes eight to twelve hours in the recipient. The donor right lobe hepatectomy in a live-related transplant takes four to six hours. The anaesthesia and critical care team remain with the patient throughout, and transfer to the transplant ICU follows immediately. Kidney transplantation is a shorter procedure and takes three to four hours. Operative duration varies based on anatomy, complexity, and intraoperative findings.

The immediate post-operative period is spent in the dedicated transplant ICU typically three to five days for a kidney transplant and five to ten days for a liver transplant, depending on graft function and the absence of complications. Ward stay follows, with daily monitoring of graft function, immunosuppression levels and signs of infection or rejection. Initial hospital discharge typically occurs two to three weeks post-operatively for liver transplant and one to two weeks for kidney transplant. The first three months after transplantation are the highest-risk period for both rejection and infection, requiring intensive outpatient monitoring. Long-term, recipients take immunosuppression indefinitely and attend structured follow-up at defined intervals.

Patients with end-stage renal disease (whether on haemodialysis, peritoneal dialysis, or approaching dialysis) who are medically fit for surgery, have no active malignancy or uncontrolled infection, and have a compatible donor are eligible. There is no fixed age cut-off, though older patients require more thorough cardiovascular assessment. Patients with significant coronary artery disease, severe pulmonary hypertension, or other conditions that substantially increase operative risk may not be suitable candidates. Eligibility is determined individually through our pretransplant evaluation.
