Eligibility & Terms and Conditions

eligibility-1 Cardholder shall be 60 yrs or above and shall hold an authorised government ID as age proof.
eligibility-1 Only one member per card can be enrolled. The Card is Non- Transferable. One UHID per card
eligibility-1 The DOB & age proof must match the address provided during billing. Expired, illegible, or mismatched documents will result in cancellation of membership and a request to resubmit.
eligibility-1 It is mandatory to present a copy of the Valid Age proof at the time of billing.
eligibility-1 No two offers can be combined under any circumstances
eligibility-1 If, at any point after activation, it is found that the registered documents do not match or are found to be fraudulent, CARE Hospitals reserves the right to immediately suspend or terminate the membership.
